Eternal Items Mod descriptions  All information about new items will appear here. You can leave comments if you sign in.  You can suggest a ​ ny​ ideas for new items below the table. All ideas will be considered.  Correction of grammatical errors is also welcome. 

 

Alcohol  "Luck brings  intoxication" 

Passive.   Tear modifier. 

Passive.   Ambrosia    "Food of the gods"  Tear modifier. 

Treasure and shop  Luck changes randomly with each shot from +0 to +10. Luck decreases accuracy.  item pool. 

Angel item pool. 

Gives 2 ethereal hearts. Isaac’s tears summon light beams from the sky when  they touch walls or enemies.  

Bag of Tricks 

Active.  6 Charge. 

Treasure, secret  item, and golden  chest pool. 

Spawns 3 random shadowed items. Each item can be a collectible, chest, pickup,  or troll-bomb. If one of the shadowed items is collected, the other two will  disappear. 

Ball Lightning 

Active.  2 Charge. 

Treasure item  pool. 

Isaac shoots lasers at all enemies and projectiles, creating chains between all  enemies and pushing away all projectiles. The lasers copy Isaac’s tear effects and  deal anywhere from ⅓ to 2 X Isaac's damage.  

The Firmament  "You take the  Moon..." 

Passive.   Orbital. 

Angel item pool. 

Slowly spins around Isaac in an unusual trajectory and deals contact damage  equal to 1/4 of Isaac’s damage. If a projectile hits The Firmament, it is blocked  and activates a huge Godhead aura that deals 1/10 of Isaac’s damage per tick.  

Black Key  "From the cabinet  of the devil" 

Passive. 

Devil, curse, key  beggar item pool. 

Some enemies drop spiked or red chests after death. Chance depends on luck. 

Blue Soap  "Slippery shots!" 

Passive.   Tear modifier. 

Treasure and shop  Chance to fire soap tears that cause confusion, makes enemies leave blue creep,  item pool.  and makes enemies shoot tears in random directions. 

Boiling Blood    "They will suffer" 

Passive. 

Devil item pool.  COST 2 HEARTS 

Some enemies bleed and leave puddles of blood. Damage does not depend on  Isaac's damage. 

Karmic Vessel 

Passive. 

Devil and curse  item pool. 

Taking damage causes the fear effect on all enemies. Isaac shoots 4 brimstone  lasers in a “+” shape that deal half Isaac`s damage and copy tear effects (e.g.  Rubber Cement). 

Broken Tech  "Just rubbish... or  not?" 

Passive.   Tear modifier. 

Treasure item  pool. 

Some of Isaac’s tears shoot lasers that copy the effects of Isaac’s tears. Laser  length scales with Isaac's shot speed. 

Passive.  Calcium  "BONES = POWER" 

Treasure and boss  item pool. 

+1 empty bone heart. Damage Up. Every bone heart gives Isaac an additional  damage up. 

Cold Blooded 

Passive. 

Devil item pool. 

Gives 1 cold heart. Isaac gains +0.03 damage per enemy killed, but gains +1 tear  delay per 75 kills. When Isaac takes damage, he has a chance to take triple  damage and reset Cold Blooded. This chance grows with the number of enemies  killed (0.5% per kill). 

Cool Shoes 

Passive. 

Treasure item  pool. 

Immunity to spikes and creep. Speed Up. When Isaac moves, nearby enemies and  projectiles are pushed away from him with a force proportional to his speed. 

Copper Tube  "Soft, but strong" 

Passive. 

Treasure item  pool. 

Tears up. Isaac’s shot speed slowly changes with each shot. Shot speed affects the  damage of tears. 

Treasure and  beggar item pool.  SPECIAL ITEM 

Chance to fire bean tears that cause shockwaves. The size of the shockwave scales  with Isaac’s damage. At 5+ luck, there is an additional chance to spawn an  explosion in the center of the shockwave. The explosion copies Isaac’s bomb  effects. Use with caution. 

Passive 

Treasure and  angel item pool. 

If Isaac does not shoot or take damage for some time, he shoots 4 holy beams in  all directions. The effect works once per room. Beams copy the damage and  effects of Isaac's tears.

Active. 

Treasure and  secret item pool.   

Can be used in an empty room or a room with enemies. After use, strange things  happen. 

Dad's Fishing 

Passive. 

Treasure item  pool. 

Luck-based chance for some enemies to drop coins, keys, bombs, chests or blue  flies/spiders. 

Passive.   Orbital. 

Treasure item  pool. 

Slowly orbits Isaac, dealing ⅓ of his damage as contact damage. When a tear or  projectile hits the sponge, it increases in size. Upon reaching critical mass, the  sponge releases 8 tears in a circle and reverts to normal. Tears copy Isaac’s tear  effects.

First Blood  “First death”

When the first enemy dies in the current room, Isaac becomes red and receives  tears and damage up, which gradually resets to zero over time. 

Flaming Stick  "Kids, don't try  this at home!" 

Passive.   Orbital. 

Treasure and  secret item pool. 

Quickly spins around Isaac and rapidly shoots fireballs at enemies. The Flaming  Stick does not block projectiles. More enemies nearby, the more fireballs shot.  Each fireball does 2 damage and does not scale with Isaac's damage. 

Gasoline  "GAS GAS GAS!!!" 

Passive. 

Treasure item  pool. 

All small friendly puddles turn into flames. Each fire deals 1/3 of Isaac's damage  per tick and blocks projectiles, but quickly go out. When Isaac takes damage, he  releases 10 flames in all directions. 

Golden Heart  "Actually made of  gold!" 

Passive.   Tear modifier. 

Treasure item  pool. 

+1 full red heart container. +1 gold heart. Isaac has a chance to fire a special coin  tear that does slightly more damage and does additional damage based on how  many coins Isaac has.  

Golden Skull  "Unleash the  Wrath of Greed" 

Passive. 

Treasure and  golden chest item  pool. 

Gives Isaac 3 golden hearts. Taking damage causes Midas touch effect on all  enemies. After that, Isaac shoots 4 golden rays in an “X” shape. Rays copy Isaac`s  damage and tear effects (e.g. Spoon Bender). 

Green Contact  "gnimoh atros" 

Passive.   Tear modifier. 

Treasure item  pool. 

Some of Isaac's tears spawn with homing effect and lose it after a few seconds.  Homing duration increases with Isaac's luck. 

Grindstone  "Sharp surprise!" 

Passive.   Tear modifier. 

Treasure item  pool.   

Isaac gets a small chance to shoot a circular saw instead of a tear. Saws fly around  the room and bounce off obstacles and enemies. Synergizes with SMB Super Fan. 

Gunpowder  "It smells like  trouble" 

Passive. 

Treasure and shop  Dead enemies can drop troll bombs. Chance depends on luck.  item pool. 

Happy Star 

Passive.   Tear modifier. 

Treasure and  secret item pool. 

Damage Up. Some of Isaac's tears turn into luminous orbitals. Orbitals copy  Isaac`s shots. Glowing tears block projectiles and home in on enemies. 

Holy Drops  "Crystal clear" 

Passive.   Tear modifier.

When Isaac’s tears touch a wall or enemy it leaves a Holy Water puddle. There is a  chance for the puddle to be larger based on Isaac’s luck. 

Laser Mind  "Ultimate laser  defence" 

Passive. 

Treasure item  pool. 

Isaac shoots a pink laser at nearby hostile tears and enemies. Tears are pushed  away from Isaac. The fire rate scales with Isaac’s Tears stat. 

Leaky Pockets  "More stuff to  find!" 

Passive. 

Treasure item  pool. 

+ 5 coins. A small chance to find a collectible or shop item in an unexplored room. 

Living Spores  "Explosive  infection" 

Passive. 

Treasure item  pool. 

Some enemies explode with splashes of tears. Tears copy Isaac`s shots. Chance  depends on luck. Synergizes with Tech.5, Technology, Technology 2,Tech X,  Brimstone, Dr.Fetus, Epic Fetus and Ipecac. 

Passive.   Magic Sponge  "Magnet for tears"  Orbital. 

Treasure item  pool. 

Slowly spins around Isaac and deals contact damage equal to 1/3 of his attack. If a  projectile hit the Magic Sponge, it is blocked and activates the sponge. The  activated sponge emits a huge aura that attracts projectiles to it. 

Meat Cleaver  "Swing Hit Cut" 

Passive.   Tear modifier. 

Treasure and devil  Tears Down. Isaac shoots cleavers instead of tears. Cleavers inflict damage like  item pool.  Mom's Knife and have many synergies with other items.  SPECIAL ITEM  COST 2 HEARTS 

Miner's Lantern  "Luck Up +  Explosion  expertise" 

Passive. 

Treasure and  secret item pool. 

  "Orbital defense +  Damage Up" 

Luck Up. A chance to ignore damage from explosions or shockwaves. Some rocks  contain trinkets and collectibles and drop them when destroyed. 

Passive.   Plastic Fork  Tear modifier.  "Weighs like a  feather, strikes like  a pitchfork" 

Treasure and shop  A chance to fire a special piercing shot. When it hits an enemy, it scatters into 3  item pool.  smaller projectiles that deal 1/3 of Isaac's damage. 

Raven's Skull  "Curse of Bones" 

Passive. 

Treasure and  curse item pool. 

Every time Isaac receives a third soul or a black heart, he converts 3 soul or black  hearts into one full bone heart. The effect works until Isaac has 6 bone hearts. 

Red Matter  "Concentrated  brimstone" 

Passive.   Tear modifier. 

Devil item pool.  COST 2 HEARTS 

There is a small chance to turn Isaac’s tears into a red orb. When an orb touches a  wall, enemy, or the floor, it explodes with a volley of tears and brimstone lasers.  Isaac's stats affect the size of the explosion of tears and distance of the  brimstone lasers. Tears and lasers copy Isaac tear effects. 

Sad Star 

Passive. 

Treasure and  secret item pool. 

Tears Up. Some of the enemy's projectiles orbit Isaac, becoming blue orbitals.  Blue orbitals block other projectiles. 

  "Orbital defense +  Tears Up" 

Pudding  "Om-Nom-nom" 

Passive. 

Treasure and boss  item pool. 

If Isaac does not have red heart containers, grants him 2 red heart containers. If  Isaac has less than 6 red heart containers, grants him 1 red heart container. If  Isaac has 6 or more red heart containers, grants him 1 black heart. Passively  grants Isaac all stats up if he has six or more red heart containers. 

Saw Blade  "Spinning death" 

Active.  4 Charge. 

Treasure item  pool. 

When used, Isaac shoots 4 saws diagonally. The saws fly around the room until  Isaac leaves it. Each saw deals damage equal to Isaac’s damage. 

Shiny Fuse  "Holy Bombs" 

Passive.  Angel item pool.  Bomb modifier. 

ALL Isaac's bombs (even from Anarchist's Cookbook) spawn golden rays in all  directions when explode. Rays copy Isaac`s damage stat and tear effects (e.g.  Spoon Bender). 

Ship's Wheel  "Drifting tears" 

Passive.   Tear modifier. 

Treasure and  secret item pool. 

Shot speed down. Range up. Isaac's tears move in the form of waves. 

Socks with Glass  "Filled with  shards of pain" 

Passive. 

Treasure and boss  item pool. 

Isaac's fire rate increases with his velocity and decreases when slowed down. Can  go above the maximum fire rate. 

Steel Fly  "Attacking  Orbital" 

Passive.   Orbital. 

Treasure and  secret item pool. 

Slowly orbits Isaac, dealing damage equal to 1/4 of his damage. If an enemy gets  close, it will start orbiting that enemy, shooting at them.  

Cursed Heart  "Blood = Power" 

Passive. 

Devil and curse  item pool. 

When Isaac collects red hearts or soul hearts, there is is a chance that he will gain  fewer hearts than he collected. Lost red hearts will increase Isaac’s damage.  

Passive.   Tech IX  "Cuts enemies into  Tear modifier.  pieces" 

Treasure item  pool.  SPECIAL ITEM 

Isaac’s tears are surrounded by a small Tech X laser ring. Rings copy Isaac tear  effects, speed and deals 1/3 of his damage. The size of the ring scales with Isaac’s  tear size. 

Thick Bones  "Speed Down +  Resistance" 

Passive. 

Treasure item  pool. 

Speed Down. +2 full bone hearts. Chance to block any damage. This chance varies  with the number of bone hearts (4% per heart). 

Vegetarianism  "All Stats Down" 

Passive.   Tear modifier. 

Treasure item  pool.  SPECIAL ITEM 

All stats down. Isaac’s tears have a chance to become explosive carrot rockets  that leave tears behind. Isaac does not receive any damage from carrot  explosions. 

Velociraptor Toy  Passive.  "Fast and Deadly" 

Treasure and boss  item pool. 

Slight damage down. Isaac's damage increases with his velocity and decreases  when slowed down. 

Wake Me Up 

Passive. 

Treasure and  secret item pool.  SPECIAL ITEM 

Doubles the health of all bosses. Duplicates all enemies. Duplicates collectibles  and pickups in special rooms, except Secret Rooms, Shops, Devil Deals and the  Black Market. 

Emerald Ring  "My precious" 

Passive. 

Secret item pool. 

Some enemies turn green. When a green enemy is killed, it creates a green laser  sphere that grows, and then decreases in size. Time of life, damage, size of the  sphere depends on Isaac’s damage. 

Amethyst Ring    "Fascinating" 

Passive. 

Secret item pool. 

Some enemies turn purple. Purple enemies are permanently charmed. 

Passive. 

Secret item pool. 

Some enemies turn red. Red enemies have 3 times more health, but with a high  chance they will drop hearts on death. 

Passive.  Familiar. 

Treasure item  pool. 

Isaac gets a blue toy car that attacks enemies and jumps over pits and rocks.  Contact damage is equal to 1/4 of Isaac damage. When it accelerates enough (this  can be seen by changed animation), it causes double damage. It does not cause  damage while jumping. 

Passive.   Tear modifier.

A chance to shoot a purple mushroom. Upon contact, the mushroom explodes  and forms a pink cloud. Cloud shoot tears with a charming effect in all directions. 

Math    "Think  differently” 

Passive.   Tear modifier.

Small chance to shoot a formula instead of a tear. The shot's damage is 

Skateboard    "Ride, but don't  break your legs” 

Active. 

Treasure item  pool. 

It charges when Isaac dodges the enemy bullets. When activated, Isaac gets on  the skateboard and start moving around the room in random directions with  invulnerability effect. Isaac is able to shoot while riding a skateboard. Each  character has unique skin for the skate! 

Dark Symbol    "Withering” 

Passive. 

Devil and curse  item pool. 

Mechanics are very complicated, in short: all stats inverted, black hearts are  good. Does not affect movement speed.    All stats (except speed) are calculated according to the following algorithm:  N = X - O  N - a new stat.  X - initially a high value of this characteristic, which decreases with the taking of  damage and increases when collecting black hearts.  O - old stats of the player (calculated without this item).    This means that this item will revive a terrible run, but destroy a good run.

Coconut Milk  "Tears Up” 

Passive.  Tear modifier. 

Angel item pool.  SPECIAL ITEM 

Tears Up. Damage Up. Shot speed down. Some of Isaac's tears appear with an  aura of light and a homing effect. When the tear with an aura hits the enemy, all  other tears on the screen turn into homing light balls aimed at the enemy, which  was touched by a tear. 

Passive. 

Treasure item  pool.  UNLOCKABLE 

Isaac gets great Tears Up, that decreases when he shoots tears. Effect restores to  maximum when Isaac travels between rooms. (​ Unlock: Kill Hush with Soy Milk.) 

Golden Bombs    "Elite explosives” 

Passive.  Treasure and  +5 bombs. Enemies and rocks, destroyed by Isaac's bombs, drop coins with a  Bomb modifier.  bomb beggar item  high chance and other pickups with a small chance.​ ​(Unlock: Beat "Collector" challenge.)  pool.  UNLOCKABLE 

Lion’s fang    "High risk, high  reward.” 

Passive. 

Treasure item  pool.  UNLOCKABLE 

Challenge rooms contain much more stuff. Waves can spawn double or triple  amount of monsters. High chance to spawn champions or giant monster versions  with 3 times more health. (​ Unlock: Clear 5 challenge rooms in one run.) 

Yummy Flesh    "Self-cannibalism ” 

Passive. 

Treasure item  pool.  UNLOCKABLE 

Isaac has a high chance to drop the piece of flesh when receiving damage. The  flesh cools down 1-2 seconds after then can be eaten to restore half the red heart.  The greater the health of Isaac, the longer the flesh lives. If flesh spoil, it will hurt  Isaac if he steps on it. (​ Unlock: Beat "Chef" challenge.) 

Silver Key  "Go beyond  reality” 

Active.  6 Charge. 

Angel and key  beggar item pool.  UNLOCKABLE 

Creates a room that does not belong to this floor and teleports Isaac into it. Luck  increases chance that the room will be special.​ (​ Unlock: Beat "The faint" challenge.) 

Fly Tears  "Something went  wrong in the lab  today...” 

Passive.  Treasure item  Tear modifier.  pool.  Bomb modifier.  SPECIAL ITEM 

Isaac's tears and bombs turn into homing flies. 

Passive. 

Treasure item  pool. 

A chance to shoot with a marked tear. This chance depends on Isaac's luck. After  contact with an enemy, the tear begins to move between other enemies,  damaging them. 20% chance to end the streak. 

Voodoo Pin  "Evil + Challenge  + DMG up” 

Passive. 

Devil and curse  item pool. 

Damage up. When Isaac enters the curse room, he gets half a black heart. All  enemies and bosses receive a health multiplier equal to “1 + Isaac's Damage /  100”. (This means that if you encounter an enemy with 20 HP and you have 15  DMG, his health will rise to 23) 

Midas’ Lament     "Golden body” 

Passive. 

Treasure item  pool. 

Isaac leaves a short trail of gold, which does not cause damage but causes the  Midas effect on enemies. Each coin converts into 0.02-0.03 damage up. 

Osteosarcoma  "Tearing Flesh” 

Active. 

Treasure and red  chest item pool. 

Always charged. If Isaac has 3 or less red hearts, removes half a red heart and  gives an empty bone heart with a 10% chance. If Isaac has more than 3 red hearts,  removes 3 red hearts and gives 1 empty bone heart. 

Cruets  "Blood of the  Lamb" 

Passive.   Orbital. 

Angel item pool. 

Grants two chalices as distant, slow orbiting familiars that deal very little contact  damage, but can create medium pools of creep when hitting enemies. Chalice  with water creates a puddle that gives Isaac Tears Up. Chalice with wine creates a  puddle that gives Isaac Luck Up. 

Seafoam Fossil  "All hail Helix" 

Passive.   Tear modifier. 

Super secret rock  drop. 

Isaac shoots 2 tears at once, but they arc out and back in with a wave pattern.  Chance to shoot out a fossilized tear, that pierces enemies and deals 1.5 x damage.  “Ship's Wheel” increases the chance to find this item by three times. 

Paten  "Flesh of the  Lamb" 

Passive.  Familiar. 

Angel item pool. 

 

Grants a large bread loaf familiar that trails Isaac. The bread blocks enemy tears.  Every time the loaf blocks a tear, there is an increasing percentage chance that  the bread will become more stale. There are multiple levels of staleness, each  level has to advance to the next from 0% to become more stale. Isaac can collect  hearts to “heal” the bread. 

 

Yoga  "Align your  Chakras" 

Passive.   Tear modifier. 

Angel item pool.  SPECIAL ITEM 

Range down, tears down, damage down. Isaac briefly flashes in a cycle of seven  colors; red, orange, yellow, green, blue, violet, and magenta, then repeats. The  number of tears Isaac shoots matches the rank of the color he is currently  glowing, from 1 to 7. The tears can fire slightly off-center. The frequency of the  flashes goes up with tears up’s. 

Voodoo Baby  "I feel his pain" 

Passive.  Familiar. 

Treasure, curse,  Familiar that follows Isaac and blocks projectiles. When struck it has a chance to  and red chest item  trigger a Dull Razor effect.  pool. 

 

Trick Shot    "Calculated” 

 

 

 

 

NUKEM    "Mega  explosions” 

Passive.  Treasure and  +10 bombs. When Isaac has 10 or more bombs, pressing bomb button will place a  Bomb modifier.  bomb beggar item  nuke on the ground that creates Mama Mega explosion in the current room.  pool. 

Lunch box    "Snack break!” 

Active. 

Treasure item  pool. 

6 charges. When used, it has a chance to spawn a random lunch item, Meat or  Jesus Juice, with the odds going from 10, 25, 50, 75%, increasing each time you  successfully get a lunch item. Using it after the 75% mark consumes the item. 

Passive. 

Curse and secret  item pool. 

25% chance ignore damage and teleport randomly when taking a hit. Standing  still for a few seconds will teleport Isaac randomly. 

 

Crystal apple  "Cursed?” 

  Your ideas  You may comment on other people's ideas (highlight post you want to comment on and select ).    Guys, I'm glad to see your ideas, but I did not expect that there will be so many! 1. Myiasis: ​"Fly Tears" ​A passive item that would give Isaac the Pop! effect and homing tears at the expense of a tears down. The tears would be skinned to look like little flies, and it would be cute if Mom's Knife was held up by a fly, and bombs from Fetus in a Jar had little fly wings. Done. 2. Yin-Yang Pendant: ​“Exchanging Civilities”​ ​A passive item that appears in both Angel and Devil deals (Possibly shops too), that when picked up, swaps the devil and angel pools. (Ex: Devil items would be free inside of angel deals, and you’d have to pay hp for angel items in devil deals) Refused, because the idea is too simple. There is already such an item called “Permutation” in Steam Workshop (​link​).

3. Voodoo Pin: ​This item removes the teeth of the curse room doors, making them free to enter. This item also gives you a +0.5 to dmg but at a cost. Enemies get a 1.(X= isaac’s dmg) multiplier to their health. Done.

4. Trick Shot: ​“Calculated” ​(Would look like a pool cue) ​Passive. ​Isaac’s tears have a chance (10%) to become homing, spectral and bounce accurately from enemy to enemy. Can bounce at most 6 times. Chance to shoot a ​“Trick Shot” ​ tear increases with luck, the cap being 100% at 17 luck. ​“Trick Shot” ​tears can bounce to the same enemy multiple times. Combos with: ​“The Mind”-​ Increases accuracy and shot speed even more. ​“Lost Contact”- T ​ ears now can bounce from destroyed tears, ​“Trick Shot” tears will now target enemy tears as well as enemies. ​“Pop!” + “Lost Contact”- ​Enemy tears hit are no longer destroyed, instead they are turned into more ​“Trick Shot” ​tears, dealing Isaac’s damage. ​Piercing ​- Bounce cap is removed. ​“Brimstone” + L ​ asers​- ​Beams end when hitting an enemy, as if it had hit a wall, (it's still bouncing brimstone tho). Done.

5. Gold: ​" Golden body "​ ​(it looks like an alchemical sign of gold, Isaac looks like pale gold) passive. Isaac leaves golden creep, which has the effect of Midas on the enemies in contact. Creep does no damage. Each coin of Isaac will be converted to +0.2 damage. Done.

6. Broken Bone​: ​" Tearing Flesh "​ Active 0 room charge. When used empties 2 red containers, gives one empty bone heart. Can not be used with one heart or if there are no red hearts and if no 2 full hearts. Done.

7. Cruets: ​“Blood of the Lamb”​ ​Angel Pool. Two golden, lidded chalices, one filled with water (marked A) and the other with wine (marked V). Grants the two chalices as distant, slow orbiting familiars that deal very little contact damage, but can create medium pools of creep when hitting enemies. The A chalice spawns blue creep that increases Isaac’s tear-rate while he stands in it, and the V chalice spawns red creep that increases Isaac’s luck by 5~ while he stands in it. If maneuvered correctly, both creep puddles can be spawned on top of eachother, granting both bonuses at the same time. Both creeps deal an identical fraction of Isaac’s damage to enemies. Done.

8. Paten: ​“Flesh of the Lamb”​ Angel Pool. Ornate golden plate on which sits a fresh loaf of bread. Grants a large bread loaf familiar that trails Isaac similarly to the Bums (lingers a bit before moving, continues to catch up even if Isaac has stopped moving, etc.) The bread blocks enemy tears. Every time the loaf blocks a tear, there is an increasing percentage chance that the bread will become more stale. There are multiple levels of staleness, each level has to advance to the next from 0% to become more stale. The bread becomes smaller and slower at each level. When Isaac is at full health, he can collect red hearts to decrease the staleness % to “heal” the bread.

9. Seafoam Fossil: ​“All hail Helix”​ Super Secret Rock Drop. Isaac shoots 2 tears at once, but they arc out and back in with a wave pattern. Chance based on luck to shoot out a fossilized tear, that pierces enemies and deals 1.5x damage.

10. Yoga: ​“Lined up tears“ A ​ ngel Pool. Charge shoots. Isaac shoots tears in full line. By line, means there will be tear, after some distance another tear, after distance another, and so goes on. Tears stay in one place for some time, after that, they disappear. It synergies​ ​with brimstone, creating red circles. Same with lasers, and other synergies.

11. Eagle Feather: ​“Use Fly!” An active item with a three room charge that launches Isaac into the sky, and makes him land on a random enemy. When he lands, He’ll deal 100 damage to the enemy in question and gain invincibility frames (for obvious reasons). Enemies won’t target you during flight or a second after landing. It will only appear in the black market or rarely in the shop/item room.

12. The Lion and the Lamb: ​“Tame the beast” A ​ ngel Pool. Active item, 6 charges. Item is profile view of both a lion and lamb’s head. When held, spawns a lamb head familiar that wanders aimlessly around the room. Similar to the Punching Bag, it can attract the attacks of enemies. When used, swaps out the lamb head for a lion head familiar, which pounces at nearby enemies, dealing considerable damage. Aside from pouncing, the lion head can barely move. After several seconds, the lion head will swap back out for the lamb head. If the lamb head takes too much damage, it will become “inactive”, prevent it from attracting enemy attacks. This will reset if the item is used or a new room is entered.

13. Red Bomb​: "Mega explosions" Passive (Looks like a red bomb with an evil skull on it, only bomb beggar and secret room pool). +5 bombs, when it explodes it destroys all the objects in the room and opens the secret room at any distance. Also does 10 damage to all enemies in the room and the effect of combustion. Explosion damage increases by 3 times. But when Isaac puts the bomb, he spends 3 bombs for one. (Isaac has 5 bombs - he puts a bomb - he has 2 bombs.) ​link

14. Isaac’s Drawings​: ‘’Just doodles’’ Passive; Tear modifier (maybe special item?). Treasure pool. Isaac has a chance to shoot a ‘drawing/paper tear.’ There are three different ‘drawing/paper tears’ Isaac can shoot indicating different items (add random sequences; no order). ​1​: A knife drawing that inflicts damage like Mom’s Knife. ​2​: A bomb drawing indicating an Ipecac shot; explodes on impact but has no arc. ​3​: A lightning bolt drawing that acts like a Jacob Ladder’s tear (add more if possible/if you want)

15. Notepad​: "You can write in it.." Active 4 rooms charge (item room or library). When activated it casts a random effect on enemies. If used in one room with another book, takes its effect (as Void). After absorbing book will use its effect too. Can absorb books: Bible, Belial, Necro, Shadows, Anarchist, Revelation, 7 sin, Manual, Telepathy, Secrets, Satanic Bible, Dead Book. Trinket Bible Tract will give eternal heart after use this item(when holds).

16. Lunch box: “Snack break!” ​Active item, 6 charges. When used it has a chance to spawn a lunch item, with the odds going from 10/100, 25/100, 50/100, 75/100, increasing each time you successfully get a lunch item. Using it after the 75% mark spawns a Meat! and consumes the item. Using it with void makes it able to spawn lunches at a 75% chance rate per use, but can never spawn Meat!

17. Black Tentacle: ​“Maddening appendage” D ​ evil Pool, 2 Hearts (?). A black, glistening thorny tentacle. A long black tentacle whips straight forward out of Isaac whenever Isaac stops firing tears. The tentacle pierces through enemies, dealing damage to all enemies whipped. Charges similarly to Brimstone but isn’t a tear modifier itself. The longer Isaac charges it, the further it can reach. Its charge rate is related to Isaac’s tear stat and its max range is Isaac’s range X 1.25~. Isaac needs to fire a certain number of tears before the tentacle starts charging, to prevent spamming unless Isaac has a really high tear stat. Inspired by the “Snapper” enemy from the Antibirth mod.

18. Positivity: ​This item would half all negative effects that you gain from other items, and it would give a 1.25x multiplier to all positive effects from other items. It would be a special item.(Tag line could be "More positive, less negative.") link

19. Mycetoma: ​“Fungal frenzy”​ ​Passive item. Tears that have a chance of shooting a tear that increases an enemy’s size and also infects them with a mushroom that does poison damage.

20.Burnt Spoon: ​“We have to cook!” P ​ assive item. Boss, Shop, and Devil Beggar pool. Drops one pill. Doubles the effect of all applicable pills, both good and bad. Stat up and down pills have doubled stats, pills that affect the floor affect the next floor, pills that affect the room affect the next room, pills that have durations have doubled durations, etc. It’s the Tarot Cloth but for pills. Some pills won’t be affected, of course.

21. Broken R Key: ​“THIS GAME SUCKS!!!”​ ​This active item would be a one time usable that teleports you to basement one with a new seed, the catch is that all of the items you had previously are now spread across all the rooms of the floors that you completed. This includes trinkets that you may have had, as well as trinkets smelted into you​. ​This item would be the keyboard key R with a crack on top. 22. I​ tem​ ​Half-Delirious​: "Partially crazy" Passive, item pool Angel and Devil room. Makes Isaac's face partially white. Isaac's tears become as Delirium tears(white). Tears have a chance to impose a completely random effect on enemies, depends on luck. Also ensures the appearance of a portal into the Void after each defeated boss. A chance when receiving damage change everything in the room(D10, D12, D6, D20 effects). ​Costume

23. Item​ Lost Deck​: "It doesn't exist" Passive, item pool Secret room and I'm Error. +1 Luck +0.50 Range +0.10 shot speed. Each card replaced by Glitch Card. When Glitch Card picked up - it a random card. If Glitch Card is dropped it changes its appearance and when picked up again is another random card. But if the Glitch Card has been lying around for too long, it has a chance of disappearing. Runes and Dice Chard are not replaced by Glitch Card. ​Cards​ ​(I hope you make their glitch animations yourself, because I don't know how to do the animation..)

24.Perfect Disguise:​ “They won’t see me coming” P ​ assive, Treasure(?)/Shop Pool. It’s a pair of Groucho Marx glasses, which Isaac wears. Gives Isaac two invisible(?) auras. The first is very large, about a quarter+ of a room big. Enemies outside of this aura have a significant chance to become confused for some time. The second aura is very small, just big enough for an enemy to be within it and not touch Isaac. Confused enemies that are within the small aura take double~ damage.

25. Horrific Eye:​ “It Scares the Depths Apart” This item would give a familiar that’s a floating, black eye that shoots tears that create ventricle razor holes in the ground. The amount of holes on the ground is capped at two, and if a third is created, it would remove the first portal so that it could spawn.If a hole created would softlock the player, then it won’t spawn. The eye shoots at a rate of every 5 seconds and every tear it shoots deals isaac’s current damage. It’s tears also give the fear effect as a bonus.

26. B ​ oard Game:​ “Reroll to level up” Active, 3 charges, Treasure/Shop/Boss(?) Pool. It’s an RPG style board game with character pieces. When used, it rerolls all collectibles in the room into new items that belong to a special “Level Up” Pool. There are nine different items in the Level Up Pool: Hearts Up “You feel healthier” +1 red heart, fills all red hearts; Armor Up “You feel sturdier” drops 4 hearts that are a random mix of soul and black hearts; Damage Up “You feel beefier” +1 damage; Tears Up “You feel gloomier” +0.5 tears; Range Up “You feel stronger” +5 range +1 tear height; Shot Speed Up “You feel snappier” +20% shot speed; Speed Up “You feel quicker” +0.4 speed; Luck Up “You feel luckier” +1 luck; Stuff Up “You feel wealthier” drops a dime, a double bomb and a single bomb, and a double key.

27. V ​ oodoo baby: ​“I feel his pain”​ Passive familiar which acts like dry/fart baby, following Isaac and blocking enemy projectiles. When struck has a chance to trigger Dull Razor effect.

28. N ​ esting doll:​ Passive item. Then Isaac takes damage he decreases in size and leaves behind a temporal decoy, similar to current size of Issac, which absorbs a certain amount of damage. Isaac’s size returns to normal after leaving a room.

29. M ​ ania:​ “I FEEL ALIVE!” A passive stat up that increases your speed by 1 (essentially just max speed), and spawns a familiar that sparratically moves around the room in different patterns (Wiggle worm, speeding up, slowing down, etc). If an enemy is killed while having this item, another familiar will spawn, though only for that room.

30. F ​ ever:​ “Burning Pain” This passive would give isaac an invisible aura that has a 30% chance to “evaporate” incoming tears. The aura does nothing to enemies, but this item also gives a 30%(can reach 60% with luck) chance to inflict burning on enemies if isaac is touched.

31. Schizophrenia: (passive) (item room /golden chest) ​“Is that real?” Isaac has a pitch black doppelganger that follows his movement but mirrored. The doppelganger deals contact damage to enemies. Ripped Soda Can (Passive) Item Room, Secret Room, and Cursed Room/Devil Room Pools] “It’s too sharp…” Upon every hit of damage, Isaac gets a mild tears up (0.7), his tears turn red, and he has a 50 percent chance to do a Tammy’s Head effect. After the first hit of damage, he leaves a trail of damaging red creep for the rest of the room.

The Doom​: ​“Pink Demon” A familiar that resembles the “Pinky” from the original doom. It runs towards enemies and bites them. Every time he bites, he has a chance to summon a friendly leper piece and has even less of a chance to summon a friendly fistuloid or a friendly small fistula piece. Only found in the devil item pool.

S ​ HEER Heart Attack!: ​“Hearts ATTACK!” Health spawned will follow you from uncleared room to uncleared room, acting like dips and damaging enemies via contact damage until the room is cleared, where they become normal hearts again. The health can be killed when acting like dips, though. Their health is restored after they become normal health. Found in devil and treasure rooms.

39. Empty Bottle: "Water - Life" Active 5 rooms charge. Fully discharged when pick up. Can be used without full charge - ​1 charge​: regenerates half red heart. ​2 charges​: regenerates half red heart and half soul heart. 3 charges: regenerates one red heart. ​4 charges​: regenerates one red heart and one soul heart. ​5 charges​: regenerates two red hearts and two soul heart. If used very many times Isaac becomes very slow and leaves yellow creep some time. This item can only be charged after clear the room. Lil Battery(pick ups), The Battery, 9 Volt, Car Battery, Habit, Sharp Plug, Jumper Cables and other items have no effect with this item. Synergy - If Isaac stands with these item in his creep(any, not yellow), he charges an item.

40. [CENSORED]​: "​This is our [​ CENSORED]​, so [​ CENSORED]​ exists​" Passive. Speed is 0.60 and cannot be increased Range +6.60 Shot speed + 0.60 Damage +9.80, all hearts are replaced by 10 black hearts. Isaac and his tears becomes covered in black a stripes with red inscriptions "​censored​". When Isaac kills an enemy, the enemy makes sounds of ​censorship ​and bursts into a bloody mess, also a 50%(if luck is 10, chance 100%) chance of creating an familiar that deals 4 damage per tick and slowly chases enemies. These minions are black a stripe with a red "​censored​" inscription, their number is infinite but they will disappear in the new room. Isaac and familiars leave a bloody trail(not a creep). ​(lobotomy reference) Refused, because [CENSORED].

41. Snapped Femur: ​“Go break a- oh” This item simply gives you the forgotten’s bone sword thing. Lilith could act like mom’s knife (In which the slash appears from incubus and the actual visual model is in front of it too), and if you’re the forgotten, I guess it would just give a 20/20 effect? 42. Bottles: ​“My dad drinks from these” Isaac’s tears are replaced with an infinite amount of green bottles full of liquid, These have a chance to pop off the cap, launching it far away into an enemy’s face. Synergizes with Lost Cork by replacing the lid with a cork and leaving a creep trail behind, with beer pushing it forward. It would look sort of like this

43. D ​ issected - Splits the player into two separate halves, similar to Lokii. Decreases fire rate substantially, but causes Isaac to be able to shoot from both halves. The halves can be damaged, but do not have individual health, meaning if either one of them takes damage then it will always subtract health from your hearts. Depending on what direction you enter a room from, your halves will walk in different directions depending on what keys you press. So for example, if you entered a room from the left, one half would go up when pressing the W key and shoot up with the up arrow key, while the other would go down and shoot down. The same goes for other directions as well, and the other half will always appear on the opposite end of the room to the other. On top of that, the item will give Isaac flight in the form of hovering. 44. Sympathy: “I didn’t mean to hurt you!”. Isaac has a chance to fire a green sympathy tear. If a sympathy tear hits an enemy, Isaac gets a huge fire rate up that slowly decreases back to normal over 2 seconds. At 13 luck, the chance to fire a sympathy tear is 50%

45. Necrosis: “Where did that bit go?”. When damage is taken, a skin piece falls off and speed is upped for that room, said skin piece leaves a puddle of damaging creep in the place it landed it​ ​. 46. spawns a familiar shaped like a ufo that fires lasers with a small to apply the shrinking effect (god’sflesh), once the enemy is is shrunk it will be abducted. after an enemy is abducted the ufo will become inactive for 1-3 rooms (scales with the enemy healt), afterwards, when the target of the ufo is an enemy of the same kind of the abducted one, it will fire a laser with the small horn effect (insta-kill) . 47. Penance: Trinket or passive. Allows angel rooms to spawn after taking a deal with the devil, but stops deals with the devil if you grab an item from angel. 48. Locked Chest: It’s fate at this point.. When taking damage for the first time on a floor the player will trigger one of six effects for the floor, each representing a stage of decay, or interpretation of his death. Changes the base color of the player, like when taking a stat-up pill. ???: Gain 3 soul hearts and a -2 tear delay, Keeper: +5 pennies and triple-shot, ItLives: A slightly weaker Brimstone, Lost: Spectral tears, Flight, Forgotten: Bone-tears, gain all effects of Negative, Soul: Spectral-tears, gain all effects of the polaroid. Cannot be stacked on the same floor unless the player took two via CrookenPenny or Diplopia allowing two effects per floor. Procs on HolyMantle hit. 49. Hedgehog Bladder “Defecate when running fast” - Isaac takes much longer to reach maximum speed. When he does, he’ll rapidly spawn poops behind him similarly to The Poop. Higher speed will make getting maximum speed take longer, but will also mean that poops spawned while running at maximum speed will spawn much more rapidly. 50. Resonance Cascade “It’s probably not a problem… probably…” - Active item which, upon use, will randomly spawn a flurry of very high damage homing green tech lasers from the middle of the room. Any enemies caught in the laser flurry will be slowed and confused. Charmed enemies will also spawn very rarely, but will not follow Isaac between rooms. The flurry of green tech lasers will eventually end after about 10 seconds. The item has a charge of 12 rooms and can only be found in I Am Error rooms. Unlocked by trying to get to the I Am Error room by using Blank Card and ? Card to get there while on The Void.

Ball of fur “why am I itchyhichy?” passive item: treasure rooms, beggars and gold chest pool. when in a room with enemies they will periodically take 10 damage and have a canche to drop half red hearts, but at the same time the player will lose red hearts (like with IV bag but won’t take away any other kind of heart). This effect will be active only if the player has red hearts containers . enemies will take damage more often once the player is at half red heart. If the player picks up the tick trinket, it will be smelted (disappears and the player obtains the effect) and the damage will increase to 20 damage. If the player has the virus effect, upon taking damage , the enemies can be poisoned and drop black hearts on death. 53. Canned meat “is this edible?” Active item 4 room charge. spawns 1-3 friendly globin goo (any variant of globin can appear) 54. Tiny chef “dinner is served” familiar that wanders around the room throwing small cleavers to near enemies. upon defeating the boss, if the player has half red heart or no red heart containers (2 or less hearts for ??? and always for the forgotten, lost and keeper) it will drop a food related item. 55. Ruined blanket “what a nightmare” active item. Grants X2 damage multiplier, tears and speed up for the current room but will also make appear a “nightmare” (acts like the shade item, imitates the player movements with a three seconds delay) that will damage the player and take away part of the bonus given by the item itself (car battery effect: double bonus and two nightmares: one with a 3 seconds delay and the other with a 6 seconds delay). If the player has the item safety blanket there is a 30% chance that the nightmare won’t appear. 56. “sealed demon” secret, challenge and curse room pool. All stats down except a speed up, the player will find himself chained with a demon (appearance similar to the dark one and the adversary). This demon will just stay sitting on the floor, blocking shots, breaking rocks and slowing down the player. once the chain connecting player and demon takes enough damage it will break, at this point the demon will either: jump and land with a shockwave, ram the enemies or shoot many balls of brimstone (like the ones from the forgotten) at high speed and rapid succession. Once the room is cleared the chain will reform, returning to the initial state 57. Juicy hearts “they’re dripping blood” red heart pickups appear bigger and dripping blood, upon pickup they may fill more heart containers than due (ex. half juicy heart fills three containers a ). this item also increh Upon taking red heart damage the player will leave a puddle of blood, a small trail of blood and release a arching blood shots in all directions that leave small puddles of blood (maybe only if you have some kind of blood related item) 58. Stuffed heart ()”pin cushion” gives a red heart container, upon taking damage will release needles in all direction that will drag and pin against the wall the enemies they hit. 59. Mutating hearts “abomination!” gives the player a rapid regeneration, if the regeneration effect procs when at full health health it will give an additional heart container, with the regeneration the player may get random stat up. if red heart damage is taken the player may lose part of its health (and even a heart container) and part of the stat up obtained from this item and spawn an “abomination”.. an abomination has the appearance and way to attackappearence of suture (small gemini, when angry) coloured like fred, that crawls on the ground and deals contact, if killed enters a goo form (like globin enemy)becomus and spawns 1-4 friendly nerve end and nerve end 2.

Black top hat “disappear!” active item that recharges every 10 seconds. spawns a pitfall (like the boss littles horn does) in the direction the player is facing , enemies that fall into this pit will disappeard (preventing other enemies to spawn on death), but doesn’t work on flying enemies. If the player has strange attractor the pitfall will attract enemies. 61. Dungeon blueprint “bonus level” sometimes, in an empty room on the floor will appear a trapdoor (similar to the crawl space one) that will lead in to a room with constant curse of darkness, filled with pits, obstacles, turrets, buttons, enemies and rewards (from pickups - easy to obtain - to items - difficult to reach ). this room can be left at any moment. 62. chance to shoot a big cluster of grey and green tears, that deals a large amount of damage, leaves a puddle of green creep that damage enemies and launches many grey/green tears and smaller clusters in all directions. 63. shoot gears that can bounce on walls and enemies 3 times (each time cracking a bit and dealing less damage), if gears touch they will move as one (resetting range, making the cracks disappear and adding the damage). 64. Shoal- charge attack that works like when you have brimstone and mom’s knife. 65. woodo market “cursed deal” passive item that can appear in any pool of the basement chapter. A room accessible only from the curse room appears, contains a shopkeeper with a hood, two “cards” are free to take:chance to give a positive effect for the floor or a 50% to give a negative effect on the floor (examples: chance for enemies to be poisoned upon entering a room, increased drop rate of pickups, higher chance for a certain kind of champion to appear, all pickups except coins are on sale), the other has a fitting positive and negative effect (example: all enemies have less health but receive double damage, all enemies lose health over time but the player may randomly drop all of his health as pickups, shops are free but coins hurt you on pickup) sometimes there is a third “card” that cost 2 hearts but always gives a positive effect (each kind of card has a different back, example: 50/50 card is purple, positive and negative is half black and half white, the positive one is golden ) . These cards have effect for one floor and upon pickup they will immediately be used and the other choices disappear. the shopkeeper can be bombed and give a black heart, spawn some spiders or flies or give one of two special trinkets: “hand of a skeleton”- allows you to pick and use the “card” as a normal card (tells you its effect or at least gives you an hint) and doesn’t make the other choices disappear; “seed of dismay” all effects of the cards will be carried across floors, can be dropped to make the effects last only for the floor (as they would normally do), if kept for 4 floors or past mom’s heart/it lives it will disappear (appears as a red coloured tree that roots on isaac head) applying its effect on the player and giving the player a damage bonus for every negative effect he has. Also an idea for a challenge: the player starts with voodoo pin, voodoo market and the seed of dismay and starts each floor from the voodoo room and the door won’t open until a “card” is taken, objective : the lamb. 66. Butt Battery: “Crappy recharge”. A passive item which gives you a 50% chance to have your active item recharged by 1-3 bars whenever you destroy a poop. Positive special variants of poop such as gold, rainbow, or black have a 100% chance to do this, while negative variants such as red or corny poop only have a 25% chance to recharge.

Banana Phone:(active, 3 charge) Upon activation, you throw the phone in any direction, and if it hits anything, a car will run it over and continue driving until it hits a wall, exploding and summoning 0-1 gapers,1-2 hoppers, 1-3 dips/flies/spiders, 0-1 horfs/maws, 0-1 skinnies, and one cyclopia, all charmed. Enemies summoned won’t travel from room to room with you. If it hits the wall or the floor, it doesn’t summon the car, but rocks and other objects like that can summon it.
